Patient Form Information
Previously, we would typically provide you with physical copies of new patient forms to be filled out in the office at your first visit. Due to new protocols, you must now fill out and submit the form online before arriving at our office.
Prior to your appointment, you will receive an email from our office with the necessary forms that can be completed through your patient portal. By completing the new patient form online, we can get your child in to see the dentist or orthodontist sooner for their examination. If you have any issues accessing the form, feel free to contact us for assistance.
